Applicant must submit a tourist voucher which can be obtained from a Hotel or Travel Agency in Belarus. This voucher must include the dates, length of stay and applicant name as it is printed on their passport.

Valid health/accidental/repatriation insurance with full international coverage. A photocopy of the front and back of the insurance card, as well as a letter from applicant’s insurance company (original or copy), is required. This letter should contain the following information:
– Name, address and phone number of the insurance company;
– Full name of the policyholder;
– Insurance policy number;
– Insurance territorial coverage (should be valid in Belarus or worldwide);
– Insurance term of validity (should be valid for the period of the travel or temporary residence in Belarus);
– The amount of emergency medical coverage (should not be less than 10 000 Euro ~ 13 500 US Dollars).
Note: A photocopy of the insurance card alone will not be sufficient enough to meet the above requirements.

Applicant must include their original, valid, signed passport. It must be valid for 6 months beyond stay. There must be at least one blank passport page available for the required Visa Stamp.

Applicant must submit (2) 2” x 2” passport photographs, front view, color with a plain white background. The photo should be recent and different from the photo used for your passport.

Applicant must submit a Letter of Invitation from a company representative of the inviting party. This Letter of Invitation may be the original, a scan or photocopy.

The letter should contain the following elements:

The date, applicants name, introduction of the applicant, detail the purpose of the trip, provide dates of the visit and be signed by a company officer with contact details including the address of the company and location where the applicant will be staying.
