Passport Renewal
Second passports can only be issued under these circumstances:
You make frequent international trips which often require entry visas and due to time constraints you have difficulty acquiring the visas before your next departure
Second passports are valid for four (4) years. The validity of the second passport can be extended at the end of the four-year period by the Department of State.
Applicant has a valid US Passport in his or her possession AND
Applicant was 16 years or older when passport was issued AND
Applicant wishes to secure a duplicate, limited validity passport.
To qualify for a passport renewal, you must be able to answer YES to all of the following conditions:
I am a U.S. citizen
I can provide my most recent passport
I was at least 16 years old when my most recent passport was issued
I was issued my most recent passport LESS than 15 years ago
I use the same name as the name reflected on my most recent passport OR I have had my name changed and can submit a marriage certificate or court decree to verify this change
You must provide your most recent, undamaged passport. Your passport cannot be frayed, torn, separating, or altered in any other way. It can be valid or recently expired. It will be returned to you marked “Cancelled” by the US State Department.
You must provide two color (2” x 2”), passport-type photographs. The photograph must:
Be printed on photo paper
Have a white background
Be two inches by two inches
Have been taken within the last six months
Taken without glasses, if applicable
The United States Passport Office has issued a guide on the correct format for photographs.
Cannot wear eyeglasses

If applicant is an infant, the eyes must be opened in the photograph.
You will need to provide one completed DS-82 Passport Renewal application form, which will be found on the U.S. Department of State website at:
https://pptform.state.gov/.
This form must be completed on-line. Hand written forms will not be accepted. This form must have an original signature
After completing the application print 2 copies of it.
This form must also be completed and signed in front of Passport
Acceptance Agency. Do not sign the application until instructed to do so by Acceptance Agent.
You will need to provide evidence of upcoming international travel as one of the following:
A computer generated flight itinerary from a travel agency or online booking company showing departure from US
If no itinerary is available a letter on business letterhead paper, addressed to the US Department of State, detailing your upcoming international travel dates and signed by your employer
Copy of Flight Itinerary
Copy of Airline Ticket
You must supply a signed letter stating why a second passport is needed and, if traveling for business matters, a second letter from your company explaining the need for a second passport.

For business travel applicants, applicant must submit a letter from their company addressed to the U.S. State Department, on company letterhead, supporting the request with the clear indication of travel plans and that the second passport is required for the applicant to fulfill their job duties signed by a company official.
For tourist travel applicants, applicant must submit a hand written letter addressed to the U.S. State Department explaining why he/she is requesting a second passport. The most common requests for duplicates are due to frequent international travel and the need to obtain visas while out of the US or traveling to countries which do not approve of another countries visa or entry stamp.
